Health Forms

Every student must return a completed Health Update form for the current school year to the Health Office.

1) You may enter your information directly on your computer.
2) Print the completed form.
3) Sign the completed form.
4) Mail or bring the signed form to the Falk Nursing Office.

For your convenience we are providing the school healthcare forms used at Falk. The five forms listed below may be viewed, printed and taken to your health-caare professional for completion. The instruction are contained in the document.

Medication Policy

This document outlines the current polcy covering the handling and adminstration of all medications for students while at school

Medication Administration Form

The Medication Administration Form must accompany any medication to be administered to your child while at school.

Dental Examination Request

Please have this form completed and signed by your dentist if your child is entering kindergarten, grade 3 or grade 7, or if your child is new to Falk School.

Medical Information Form

The Family Notebook Form will be used to contact parents, emergency contact, and medical personnel. Please be sure this information remains current. The Medical Information 2007/2008 form will be mailed to every family during the summer an should be returned to the administrative office (NOT THE SCHOOL NURSE)
This form is available on the website in the event of loss.

Physical Examination Report

Please have this form completed and signed by your child's health-care provider if your child is entering kindergarten or grade 6 or if your child is new to Falk School.
